The qSOFA screen yields a score of 2 based on altered mental status and tachypnea. Despite initial intravenous fluid resuscitation, the patient's MAP remains at 60 mmHg, and repeat laboratory testing reveals a serum lactate level of 3.2 mmol/L. When inputted into the calculator, these combined indicators confirm the presence of septic shock under Sepsis-3 criteria, immediately prompting aggressive vasopressor therapy and intensive care admission.

Always verify that serum lactate is drawn concurrently with blood pressure assessments to ensure temporal accuracy. Remember that qSOFA is designed primarily as a quick bedside screening tool for non-ICU settings, whereas the comprehensive SOFA score provides a deeper evaluation of multiorgan involvement.
