Based on standard clinical grading scales: the respiratory rate of 22 yields 2 points; the heart rate of 115 yields 2 points; the systolic blood pressure of 95 mmHg yields 2 points; the elevated temperature yields 1 point; normal urine output yields 0 points; and an alert AVPU yields 0 points. Summing these values gives a total MEWS score of 7 (2 + 2 + 2 + 1 + 0 + 0), which immediately flags the patient for urgent physician evaluation.
